Entering Results and Team Lists
The Hockey ACT Revolutionise Sports entry point can be accessed here.
Team lists should be finalised prior to each game and results entered as soon as possible following the completion of each game.
You will need a registered username in order to access this system. Please contact the club secretary if you do not have a username.

Injury and Incident Reporting Procedure
The following types of Incidents should be reported to the College via the online reporting system (Click here):
Injuries - Physical Harm including:
Notifiable incidents - Those causing death, serious injury or a "dangerous incident".
Please Note - This Incident Form is for the use of club coaches and/or managers only. For an incident report to be valid for both the College and insurance purposes, they must be submitted by a designated club member.

- Loss of consciousness
- Bone Fracture, sprain
- Dislocation
- Cuts or punctures where blood flows freely (e.g., Stitches may be required)
- Teeth are loosened or knocked out
- Ambulance is required to attend
- Injured person stretchered from the field of play
- Student's parent/guardian is advised by a Teacher/Coach/Manager to seek further medical assistance

The College holds a SchoolCare insurance policy with Catholic Church Insurance which covers all College students for College related activities within Australia and overseas. The SchoolCare claim form or the facility to make an online claim is available at www.ccinsurance.org.au. For further information in relation to insurance claims please contact the College Finance office.
